Role
- Lipid emulsion may be considered in cases of local anaesthetic induced cardiac toxicity, in conjunction with standard resuscitative measures.
- There is little evidence to support lipid emulsion use in the treatment of other forms of poisoning (e.g. beta-blocker, calcium channel blocker).
Consultation with a clinical toxicologist is recommended through local toxicology service or Poisons Information Centre 13 11 26.
Dose
Give 1.5mL/kg 20% as a bolus IV over 1 minute then 0.25mL/kg/min for 30 minutes or until haemodynamic stability is restored.
Stocking recommendations
Lipid emulsion is readily available for other uses in the hospital including parenteral nutrition. Specific stocking for use as an antidote is not recommended given its limited indications for use.
Precautions
Lipid emulsion has the potential to interfere with other life-saving treatments, including clogging dialysis and ECMO circuits and interfering with subsequent laboratory assays.
